Before Your Consultation
Complete a Health Appraisal
Prior to your appointment, you will be asked to complete a Health Appraisal. This questionnaire (which can be done on-line or as a paper copy), asks you many questions about your health and will take ½ to ¾ of an hour to complete. It provides the practitioner with indepth knowledge of your health issues and subsequently allows your consultation to be more specific. Please ensure the questionnaire is completed 24 hours prior to your appointment.
To complete a Health Appraisal Questionnaire:
- visit www.metagenics.com.au
- Click on "Health Appraisal"
- enter the following practitioner number: 39812
If you do not have internet access, please ask for a form to be sent out to you.
NOTE: Consultations can be conducted without completion of this form, but may not be as thorough.
Preparing for tests
- If you have any digestive problems, you may be asked to provide a urine sample to do a Urinary Indicans Test. Preparation required is nil alcohol for 24 hrs and some protein food the previous evening.
- Urinalysis is performed on most first consultations. There is no preparation required for this.
- Cholesterol and Fasting Blood Sugars require nil food or drink (except water) for 12 hours prior.
Cancellations and failure to attend
Please notify the clinic if you are unable to attend your appointment. This will allow another client to be slotted into that appointment time.
Failure to notify or attend an appointment, without good reason, may incur a $20 fee.
